Objectives This is a protocol for Cochrane Review (intervention). The objectives are as follows: To determine the efficacy and safety of 6?thioguanine nucleotide (6?TGN) metabolite?based dosing strategy maintenance remission using azathioprine or mercaptopurine in Crohn's disease ulcerative colitis.

Severe pulmonary toxicity after azathioprine/6-mercaptopurine initiation for the treatment of inflammatory bowel disease.

Azathioprine and 6-mercaptopurine (6-MP) are effective in inflammatory bowel disease (IBD). However, between 10% and 29% of patients treated with these drugs are forced to stop therapy due to side effects. Pulmonary toxicity due to azathioprine/6-MP has been reported infrequently. We describe 3 patients who developed severe, noninfectious pulmonary toxicity within 1 month after the initiation o...

Efficacy of 6-mercaptopurine treatment after azathioprine hypersensitivity in inflammatory bowel disease.

AIM To investigate the efficacy of 6-mercaptopurine (6-MP) in cases of azathioprine (AZA) hypersensitivity in patients with inflammatory bowel disease. METHODS Twenty nine previously confirmed Crohn's disease (CD) (n = 14) and ulcerative colitis (UC) (n = 15) patients with a known previous (AZA) hypersensitivity reaction were studied prospectively. The 6-MP doses were gradually increased from...

Effect of allopurinol on clinical outcomes in inflammatory bowel disease nonresponders to azathioprine or 6-mercaptopurine.

BACKGROUND & AIMS Many IBD patients not responding to azathioprine (AZA) or 6-mercaptopurine (6-MP) preferentially metabolize 6-MP to 6-methylmercaptopurine (6-MMP). We describe the use of allopurinol in AZA/6-MP nonresponders to deliberately shunt metabolism of 6-MP toward 6-thioguanine (6-TGN) and improve clinical responses. METHODS Twenty outpatients who were AZA/6-MP nonresponders and had...

Azathioprine pancreatitis in inflammatory bowel disease and successful subsequent treatment with mercaptopurine.

The efficacy of azathioprine and mercaptopurine is well established in the treatment of active Crohn’s disease and as a long-term maintenance therapy to prevent relapse in both Crohn’s disease and ulcerative colitis.1 Approximately 15% of patients treated with azathioprine or mercaptopurine have adverse reactions. While some of these reactions are dose-dependent, others, including pancreatitis,...

6-Thioguanine nucleotide accumulation in erythrocytes during azathioprine treatment for systemic connective tissue diseases: a possible index for monitoring treatment.

BACKGROUND Owing to adverse pharmacokinetics, azathioprine treatment may fail to induce a satisfactory clinical response in systemic connective tissue diseases. The major intracellular cytotoxic metabolites of azathioprine are 6-thioguanine nucleotides (6TGNs). METHODS To assess whether the erythrocyte accumulation of 6TGN is a clinically applicable index for monitoring azathioprine treatment...
